Most appointments are available the same day, based on patient availability. We strive to make the process efficient while maintaining appropriate medical standards of care.
How it works:
Call our office first. We will ask a brief set of screening questions to determine whether an evaluation may be appropriate.
Complete a secure intake form. If appropriate, you’ll be guided to complete our HIPAA-secure online JotForm.
Meet with a licensed provider. Your evaluation is completed via real-time, interactive audiovisual telemedicine using a HIPAA-compliant platform.
Please note: Payment is for a medical evaluation only. Certification is not guaranteed and is determined solely by clinical judgment and applicable legal requirements.
If you call and we are unable to answer immediately, we are assisting another patient.
